In a group project of two, I wrote an evaluation letter stating an employee was not performing at an acceptable level, and my classmate created an evaluation form for that same employee.

As your co-supervisors we perform a six month regular evaluation on our employees.  During the first six months of working you have been very flexible in your work hours, and you have come to work on time when we have asked, even on your time off. In addition, you are a reliable member of our staff and we are pleased to see the improvement you have made. Thank you for taking responsibility for certain situations and actions that you have done.

As a medical assistant one of the major traits to have is being able to handle stress calmly and make decisions wisely in a timely manner. This is something we will need to work on. In stressful situations it is a good idea to take deep breathes and clearly think about the correct procedures to follow. We can understand the difficulty of working at the hospital, however, we as a team need to stay positive and quick for the patients. Also, we believe that you require more training in order to continue working in this field. We offer an optional ninety day training period for our employees that we would like you to consider. Once you complete this training, you will have more knowledge of how to handling patients as well as more medical knowledge.
